What is Advanced care planning?
A process of think/talking/planning for future health care and end-of-life.
Gives opportunity to talk to family, loved ones and health care workers
Benefits of ACP
For the person: lessen anxiety over future, peace of mind, less strain on family
Loved ones: Know persons choices they would’ve made
Health Care workers: feel more comfortable providing care that works with ones wishes
Do people want to talk about ACP
Yes usually, most aren’t uncomfortable discussing EOL issues, and some have accepted their own mortality.
What stops us from making an ACP
- fears
- beliefs and attitudes
- skills and abilities
- environment
Is ACP legally binding?
No, and can only be actioned when the person is unable to take part in a medical decision. Reachable in any hospital in NZ

AD: consent or refusal to specific treatments, which may be offered when the person no longer has capacity
Clinician must be confident the patient is: informed, competent, voluntary
