Constipation Flashcards
Groups of drugs used for constipation (4 groups)
Prokinetic drugs
Muscarinic receptor antagonists
Laxatives
Serotonin agonists
Serotonin agonists - MOA (for constipation)
Promotes motility by activation of 5-HT4 receptors causing release of ACh from intrinsic submucosal neurons.
Prokinetic drugs (1 drug)
Metoclopramide
Metoclopramide - indications
GERD, diabetic gastroparesis, antiemesis.
Metoclopramide - MOA
Block dopamine D2 receptors, releasing ACh.
Increased motility and resting pressure at lower esphageal sphincter.
Blocks D2 and 5-HT3 receptors in CTZ
Metoclopramide - contraindications
Pt with seizures, mechanical GI obstruction or hemorrhage, pheochromocytoma
Metoclopramide - adverse effects
Seizures, hyperprolactinemia.
Muscarinic receptor antagonists (5 drugs)
Atropine Hyoscyamine Dicyclomine Scopolamine Pirenzepine
Which muscarinic receptor antagonist is used in PUD?
Pirenzepine
Muscarinic receptor antagonists - indications
Relief of intestinal cramping and pain.
Laxatives - groups
Bulk-forming laxatives
Surfactant laxatives
Osmotic laxatives
What are the safest types of laxatives?
Bulk-formin laxatives
Bulk-forming laxatives (2 drugs)
Psyllium hydrophilic mucilloid
Calcium polycarbophil
Bulk-forming laxatives - MOA
Absorb and retain water, increasing the mass of the intestinal material. Mechanical distention stimulate peristalsis
Bulk-forming laxatives - indications
Chronic constipation.
Diarrhea
