Congenital Heart Disease in Adults Flashcards
Common congenital defects in adults (in descending order of frequency)
Bicuspid aortic valve
ASD
VSD
Aortic coarctation
Presentation of bicuspid arotic valve
Often undetected at birth
Intense exercise accelerate complications
Complications of bicuspid aortic valve
Aortic stenosis
Aortic regurgitation
Types of ASD
Ostium secundum
Ostium primum
Osteum secundum definition
ASD high in atrial septum
Most common
Ostium primum definition
ASD
Opposing the endocardial cushions
Presentation of primum ASD
Presents early
Presentation of secundum ASD
Often asymptomatic until adulthood
Signs of ASD
AF
Raised JVP
Pulmonary HTN
Pulmonary / Tricuspid regurgitation
Mx of ASD
Closure if:
- Child
- Symptomatic adult
Causes of VSD
Congenital
Acquired (post MI)
Signs of VSD
Smaller hole = louder murmur
Harsh pansystolic murmur left sternal edge
Complications of VSD
Aortic regurgitation
Infective endocarditis
Pulmonary hypertension
Mx of VSD
Medical - many close spontaneously
Surgical
Indications for surgical closure of VSD
Symptomatic
Failure of medical therapy
Infective endocarditis
