Q

Why process into compounds

A

Enables NHSBT make effective use of donations
Allows leucodepletion of products to <5x10^6 / L WBC
Enriches constituents
Better storage of individual components
Specific products used for specific problems

Q

Manufacture of red cells and FFP

A

60% of Donation bled into whole blood filter pack
Pack 1 - CPD anticoagulant
Pack2+3- empty, 1 white cell filter
Pack 4- SAGM

Q

BD filtered into empty pack= leucocyte depleted whole blood
Donation centrifuged hard at 4C
Placed into opitpress blood seperator

A

Optipress squeezes pack = remove plasma to empty pack= FFP <8 hours men. bin>8hours leaving red cells
SAGM drained to pack containing red cells

Plasma blast frozen to -40C = FFP. If <8 hours since donation and male
Red cells stored at 4C

Q

Manufacture of red cells + platelets

A

Donations bled into bottom + top bags BAT
40% donations bled
4 packs, top of donor, 2 packs joined together “train” form from bottom of one - contains SAGM

Q

Donation centrifuged softly. = coat layer containing white cells + platelets
Placed in optipress blood separator
Squeezes pack to remove plasma into empty pack = discarded

A

+ remove red cells via leucocyte filter into empty pack, SAGM drained in = red cells stored at 4C
Left with buffy coat in original pack (platelets + white cells)

Q

Making platelets from buffy coats

A

4 joined together = train = pooled with 200ml male plasma of same group into bag
Centrifuge gently = platelet rich plasma (PRP)
Optipress squeezes PRP via leucocyte filter into final pack.
Stored at RT with agitation

Q

Manufacture of cryoprecipitate

A

Cryo = plasma product
Contains factors VIII + fibrinogen
Male frozen plasma thawed at 4C up to 18 hours
Cryoprecipitate formed
Excess plasma removed, remaining cryo redissolved
Pool of 5 donations

Q

Last step

A

All components weighed + stored in quarantine
When donation testing complete products labelled with blood group, expiry date + phenotype info
Failures discarded, donor follow up
Issued to hospital
