What is compartment syndrome..
Blood vessels and nerves run along the fascia around the muscle. The fascia does not stretch, the blood vessels and nerves become compressed
Anything that puts pressure inside or outside of the compartment that compresses the nerves, BVs
5 Ps
- Pain
- Paresthesia
- Paralysis
- Pallor
- Pulselessness
First P
Pain - Deep throbbing, pressure like
Pain is made worse with passive stretch
2nd P
Paresthesia - loss of sensation
Compression of the nerve
3rd P
Paralysis
Nerve compressed
“limb giving out.”
4th P
Pallor
Tissue actively dying
5th P
Pulselessness
Last sign - usually gone limb
Intercompartmental pressures normal
<10mm
Pressure 20-30mm
Careful observation
Pressure >40 mm
Likely surgical decompression
Tx for compartment syndrome
Remove external pressure
Place at level of the heart
Remove ice - further vasoconstrict
5 causes of Internal compartment syndrome
- Bleeding - injury
- Edema - Injury
- Insertion of external substances (high pressure pain or grease gun)
- Recent surgery
- Tissue damage - crush injury
3 Causes of External compartment syndrome
- Circumferential casts, splits etc
- Circumferential burns causing inelastic skin around the muscle compartment
- Vascular or arterial blood loss w/ hematoma putting pressure on the compartment
