communications management plan
is a subsidiary plan of the project management plan documenting communication requirements and how proper communication will be achieved. it’s purpose is to describe how stakeholders will exchange information but does not contain information about the project (description of work, schedule, budget, ext.)

the process of deploying necessary information to project stakeholders in accordance with the communication management plan
this is simply the process of following the communication management plan throughout the project lifecycle
