Weak and tight structures
Upper cervical extension with lower cervical flexion
Increased thoracic kyphosis (compresses discs)
Where is pain felt in facet dysfunction?
C2-3 to C6-7 (asymptomatic)
What are symptoms of facet dysfunction?
Sharp pain
Protective postures
Unilateral
Aggravated with movement
What is cervical spondylosis considered?
Osteoarthritis in the neck
What age do almost all people have cervical spondylosis?
40 (asymptomatic)
What causes the majority of symptoms in the upper cervical spine?
Soft tissue
Where is a common site of muscular pain in the neck?
Attachment of upper traps and sub occipital
What is osteoarthritis pain due to?
Capsular thickening and limited joint motion
What are the most common syndromes in the upper cervical spine?
Headaches
RA
Acute wry neck/joint locking
What is a joint that is commonly responsible for cervical headaches?
C3 dorsal ramus
What is occipital neuralgia in cervical headaches?
Compression of the greater occipital nerve
When does RA appear?
40-50 y/o
What is a major feature of RA?
Synovitis
What is synovitis?
Synovial membrane becomes inflamed and joint becomes swollen
What do you never want to do with cervical RA?
Manipulate
What is acute wry neck?
Stiffness between C2-3 that is characterized with an antalgic hair of slight flexion and side bending away from pain
What population commonly has acute wry neck?
Children or young adults (worst in morning)
What are the 2 different types of wry neck?
Facet or disc
What is often the cause of pain in cervical joint locking?
Facet joint
What are the common syndromes in the lower cervical spine?
Cervical spondylosis
Cervical intervertebral disc
If peripherilzation occurs with retraction repeated movements in the cervical spine then what do you do?
Move to traction (dose until symptoms stop centralizing)
