Q

What are the steps of neurotransmitters signals?

A
  1. Extrications signal travels down the axon
  2. Neurotransmitter molecules are released into the synapse
  3. Molecules bind to the receptor site
  4. The signal is picked up by the second neuron and is either passed along (excitatory) or halted (inhibitory)
  5. The signal is picked up by the first neuron resulting in reuotake of excess neurotransmitter

Q

Where is the limbiv system located and what does it control

A

Bellow the cerebrum above the brain stem
It affects emotions, behavior, motivation, long term memory, and smell. It also houses the receptors that modulate depression

16
Q

What are the known SSRIs?

A

Fluoxetine, sertraline, and citalopram

17
Q

How do SSRIs work?

A

They block 5Ht (seratonin) by binding at reuptake cite and leads to a higher serotonin concentration

18
Q

When are therapeutic benefits of SSRIs seen?

A

10-20 days

19
Q

What are the AEs of SSRIs

A

Sexual dysfunction, GI, CNS, and weight loss then gain

20
Q

What are the very common side effects of SSRIs

A

Agitation, sleep and sexual dysfunction

21
Q

What are the signs of serotonin syndrome?

A

Agitation, confusion, tremor, hyyperreflexia, shivering, diarrhea, and fever

22
Q

How soon after starting SSRIs can serotonin syndrome start

A

2-72 hours

23
Q

What is the washout period between MOAIs and SSRIs to avoid serotonin syndrome?

A

At least 5 half lives which is usually five weeks

24
Q

What other meds should you avoid with SSRIs that can cause serotonin syndrome

A

MAOIs and lithium, TCas, atypical antidepressants, triptans, and analgesics

25
What drugs do fluoxetine specifically affect and why?
It has a long half life and can affect warfarin , TCAs and lithium
26
What are the signs of SSRI withdrawal?
Dizziness, electric shock, fatigue, headache, nausea, sweating, insomnia, tremor, confusion, and nightmares
27
How long should you taper SSRIs
2-4 weeks
28
What should you add while on a SSRI taper ?
Fluoxetine low dose
29
What drug is recommended for 1st like unipolar depression treatment
SSRIs
