CLIPP 07: 2-hour newborn with respiratory distress Flashcards
Most common cause of respiratory distress in premature infants?
RDS- caused by deficiency in lung surfactant
Maternal characteristic that predisposes infants to RDS?
Diabetes
What is transient tachypnea of the newborn?
Tachypnea resulting from delayed clearance of fluid from the lungs
What defines prolonged ruptured membranes?
> 18 hour
The incidence of major malformations in a baby of a diabetic mother is directly related to maternal glucose levels during which trimester
1st
Infants born to women with HbA1c levels > 12% have at least a 12-fold increase in major malformations
When do most cardiac defects tend to present with tachypnea?
6-8 weeks
VSD, PDA, AVC
Definition of LGA
> 90th percentile birth weight
Minimum age for ordering a blood culture?
4 days and older
RR above what may require feeds by NG and IV fluids in place of PO feeds?
80
Target glucose level in a newborn prior to routine feeds?
> 45 mg/dL
Fetal blood glucose relative to maternal serum levels?
2/3rds
Why are infants of diabetic mothers are at risk for significant hypoglycemia after birth?
Their own pancreatic beta cells are stimulated to overproduce insulin in response to high blood glucose levels while being fed by mom’s blood
Treatment of asymptomatic baby with hypoglycemia?
If the baby is symptomatic from hypoglycemia, an intravenous infusion of dextrose should be started immediately as it guarantees a stable source of glucose
3 risk factors for hip dysplasia?
family history
female (9:1)
breech position
How much vit D supplementation?
400IU daily until they are weaned to cow’s milk
breast fed only- formula contains vitamin D
