What is clinical reasoning?
A systematic and cyclical process that guides clinical decision making, particularly in unpredictable, emergent and non-routine situations, and leads to accurate and informed clinical judgements.

What is stage one of the clinical reasoning cycle?
Look. Consider the patient situation. Initial impression of the patient.
What is stage two of the clinical reasoning cycle?
Collect cues/information. Review current information (medical records, etc) and gather new information (undertake assessments).
What is stage three of the CRC?
Process information. Interprets the cues that have been collected and identify significant aberrations from normal. Distinguish relevant from irrelevant information. Discover patterns and relationships between information.
What is stage four of the CRC?
Identify problems/issues. the nurse synthesises all of the information that has been collected and processed in order to identify the most appropriate nursing diagnoses
What is stage five of the CRC?
Establish goals. describe what you want to happen, a desired outcome and time frame. SMART goals.
What is stage six of the CRC?
Take action. The nurse selects the most appropriate course of action to achieve the goals of care and address the nursing diagnoses. The nurse also decides who is best placed to undertake the interventions, and who should be notified and when.
What is stage seven of the CRC?
Evaluate outcomes. Evaluate the effectiveness of outcomes and actions. Ask: ‘Has the situation improved now?’
What is stage eight of the CRC?
Reflect on process and new learning. Contemplate what you have learnt from this process and what you could have done differently..
