A

-MHx
-SHx if relevant
-DHx
(Risk status, x-rays, previous visit and purpose for visit)
(History of FTA, RA/GA, restorations, exo, dental trauma)
-How patient behaved/coped with previous treatment
-Anything that requires monitoring

A

Who they are with
What are they doing (curled in parent’s lap, running around, reading, etc.)
Do they seem happy or worried?

What info should be acquired once child is seated?

A
  • Chief Concern
  • MHx (new form for very visit)
  • DHx at other clinics
  • Clinic exam
  • No need to probe unless find suspicious areas
  • SHx (diet, OH habits, Fluoride, looked after by…)–> do this after exam to avoid child sitting too long
