Classification of perio: summary Flashcards
What is the BPE?
It is a screening tool to reach a provisional diagnosis
Who should have a BPE recorded?
All new patients
Which probe is used to take BPE?
BPE WHO probe
What BPE codes represent healthy gingiva or gingivitis?
0, 1, 2
What BPE codes suggest periodontitis?
3 or 4
What probing depths would you expect for code 3 and code 4?
Code 3: 3.5 - 5.5mm
Code 4: over 5.5mm
How deep are the pockets in code 0, 1 and 2?
Less than 3.5mm
What would you expect to see if a sextant has been scored code 0?
Pockets less than 3.5mm
No calculus or overhangs
No BOP
Black band entirely visible
What would you expect to see if a sextant has been scored code 1?
Pockets less than 3.5mm
No calculus/overhangs
BOP
Black band entirely visible
What would you expect to see if a sextant has been scored code 2?
Pockets less than 3.5mm
Supra o sub gingival calculus/overhangs
Black band entirely visible
BOP
What would you expect to see if a sextant has been scored code 3?
Pockets 3.5-5.5mm
Black band partially visible
BOP
Supra or subgingival calculus
What would you expect to see if a sextant has been scored code 4?
Pockets over 5.5mm
Black band not visible
BOP
What does * mean in a BPE score?
Furcation involvement
What percentage of BOP suggests localised gingivitis?
10-30%
What percentage of BOP suggests generalised gingivitis?
over 30%
What further investigations do you do if a code 3 or 4 is recorded?
Take radiographs
Record pocket chart (6ppc)
What elements are included in the diagnosis of periodontitis?
Extent Staging Grading Current disease status Risk factor assessment
What does staging measure?
Interproximal bone loss
What does grading score represent?
% bone loss / patient age
What stage represents early/mild perio - and what does this mean?
Stage 1
Less than 15% (or 2mm attachment loss from CEJ)
What stage represents moderate perio - and what does this mean?
Stage 2
Coronal third of root
What stage represents severe perio - and what does this mean?
Stage 3
Mid third of root
What stage represents very severe perio - and what does this mean?
Stage 4
Apical third of root
What does Grade A represent?
<0.5
Slow rate of progression
What does Grade B mean?
0.5-1.0
Moderate rate of progression
What does Grade C mean?
More than 1.0
Rapid rate of progression
What are the possible grade scores when grading perio disease?
A B C
How many stages are involved when staging perio disease?
1, 2, 3, 4
What would you include in your diagnosis statement in your notes?
Extent - Periodontitis - Stage- Grade- Stability - Risk Factors
What would you include in your diagnosis statement in your notes?
Extent - Periodontitis - Stage- Grade- Stability - Risk Factors
e.g. generalised perio stage 3 Grade B - currently unstable - Risk: smoker 15/day
