What is CLL?

What are the characteristics of CLL?
ONLY in Adults (>40)
Long course
Indolent, but incurable
What is the definition of CLL?
Malignant proliferation of lymphocytes in blood and bone marrow
What are the cellular characteristics of CLL?
aka Small Lymphocytic Lymphoma Small, mature lymphocytes B-cell with CD19, CD20, CD21, but also has CD5 Long, but inexorable course Negative for TdT
What are the clinical findings of CLL?
Asymptomatic at first
Organ infiltration later
What are the laboratory findings in CLL?
HIGH WBC
Hypogammaglobulinemia
What is the pathophysiology of CLL?
Bcl-2 gene rearrangement
*normally, Bcl-2 prevents apoptosis
What is the treatment of CLL?
Conservative
Reduce organomegaly and risk for infections
What is the prognosis of CLL?
Normal platelet count, WBC count
Adenopathy
Bone Marrow pattern
Chromosomal abnormalities
What is the cause of death in a patient with CLL?
Infection (survival is about 9 years)
What are the characteristics of hairy cell leukemia?
Hairy cells
Splenomegaly without lymphadenopathy
Pancytopenia
TRAP+
What is pancytopenia?
Low RBC, Low WBC, Low Platelets
What are the clinical findings of hairy cell leukemia?
Occurs in adults (>40)
Male:Female ratio: 5:1
What is the cytochemistry stain used to diagnose hairy cell leukemia?
TRAP+
What is the immunophenotype to diagnose hairy cell leukemia?
B-cells with CD25 and CD11c
Negative for CD5
