what is chronic limb ischaemia?
A
- peripheral arterial disease that results in symptomatic reduced blood supply to the limbs
- caused by atherosclerosis

- smoking
- hypertension
- hyperlipidaemia

what are the stages of PVD?
A
- assymptomatic
- intermittent claudication
- ischaemic rest pain
- gangrene

buerger’s test
A
- lying the patient supine and raising their legs until they go pale
- angle at which limb goes pale is named buerger’s angle
- <20 degree indicates severe ischaemia

intermittent claudication
A
- pain occurs on exercise
- absolute claudication distance - distance of walking before the pain starts
- progress to limb ischaemia
- peripheries are cold/buerger’s positive

critical limb ischaemia
A
- rest pain for >2 weeks
- ABPI < 0.5
- presence of gangrene

- ABPI
- normal > 0.9
- mild (0.8 -0.9)
- critical limb ischaemia < 0.5

- statin - 80 mg Atovastatin
- antiplatelet - 75 mg of clopidogrel
