what is acute limb ischaemia?
A
sudden decrease in limb perfusion
leads to rapid ischaemia

causes
A
1. embolisation - thrombus travels distally (AF/MI) 2. thrombosis in situ (chronic limb ischaemia) 3. trauma

clinical features
A
- Pain
- Pallor
- Pulselessness
- Parasthesia
- Perishingly cold
- paralysis
( sudden onset on these symptoms)

Ix - first line
A
- Doppler US

Initial mx
A
- surgical emergency
2. heparin infusion

conservative mx
A
- if limb is only minimally affected ( no motor loss/pulses are felt)
- Mx - prolonged course of heparin / thrombolysis with streptokinase

Surgical Mx
A
- embolectomy
2. intra arterial thrombolysis

main complication
A
- reperfusion injury - sudden increase in blood supply
- release of K+ and H+ from damaged cells causing
hyperkalaemia / acidosis - myogllogin - sig AKI
- Compartment syndrome
