Chemotherapy Flashcards
what are the goals of chemotherapy?
To cure, increase mean survival time, and decrease risk for life-threatening complications
how can cancer spread through the body?
lymphatic vessels
what is the function of chemotherapy agents?
to kill the cell
cell cycle specific dosing
schedule dependent; multiple, repeated dose
cell cycle nonspecific dosing
dose dependent; single, high dose bolus
chemo affects what type of cells in the body in terms of side effects?
cells that replicate the fastest
four common side effects of chemotherapy
alopecia, stomatitis (oral lesions), severe N/V / gastritis, bone marrow suppression
what is the best med class to prevent N/V?
5HT3 blockers; -etron family (ondansestron, etc)
most serious ADR of all chemotherapy
pancytopenia
anemia
decrease RBC, risk for bleeding and tissue hypoxia
neutropenia
decrease in WBC, risk for infection
thrombocytopenia
decrease in platelets; risk for bleeding and bruising
neutropenia
neutrophil count less than 1500
nadir
lowest point of WBC
Induction therapy
first stage of chemotherapy; attempt to induce a remission, aggressively seeking to destroy cancer cells in blood, marrow, and organs
intensification
stage 2 of chemotherapy; high dose therapy given after induction for months
consolidation
stage 3; start after remission achieved, attempt to eliminate any remaining cancer cells
maintenance therapy
stage 4; low dose chemo treatment, given q 3-4 weeks for prolonged period; goal = maintain remission
plant alkaloids are toxic to what body system?
nervous system; nurse must do thorough neuro assessment at shift change and during shift
antineoplastic agents are toxic to what?
heart; monitor total doses given to check for toxicity
methotrexate
folate antagonist; help stop reproduction of cells (normal and malignant)
MTX antidote
leucovorin- help normal cells recover and prevent anemia
common plant alkaloids
vincristine and vinblastine
common antineoplastic antibiotics
doxorubicin
breast cancer treatment
tamoxifen
BPH/ prostate cancer treatment
finasteride
angiogensis inhibitor MOA
shrinks tumors by restricting blood supply
common med = bevacizumab
filgrastim
increase WBC
ertyhropoietin
increase RBC
oprelvekin
increase platelets
what is the preferred IV site for chemotherapy?
central line
