Top right of the plate. It is written with first 2 digits as the year and last 3 digits as the day of that year. It indicates when the plate was last revised for any reason (think added obstacles, spelling corrections, etc).

Where can you find the date of the latest amendment for a procedure, and what does that date indicate?
Bottom left of the plate. It represents the most current amendment of a given procedure (changes to the procedure itself, eg. course, fix, alt, minima, etc)

ATC radar must be available to assist the pilot when transitioning from the en route environment.
