Charcoal indications

A

Adult: poisoning/OD (s-134)
- if ingestion of one of the following and is within 60 minutes.
Acetaminophen, colchicine, beta blockers, calcium channel blockers, salicylates, valproate, oral anticoagulants (including anticoag rodenticides, Paraquat, amanita mushrooms (if not vomiting)

PEDS: poisoning /OD (s-165)
- if ingestion within 60 minutes and recommended by poison control center.

Charcoal special questions:

( concentration: 50 Gm / 240 ml)

  1. Assure patient has a gag reflex and is cooperative.
  2. Do not use sorbitol / charcoal mixture because of potential for dehydration, hypotension and electrolyte disturbances.
  3. Contraindications are due to marginal effect and potential to cause harmful vomiting or burns.
  4. Use with caution with OD of drugs known to cause rapid onset of seizures (tca’s etc. ) due to risk of vomiting and aspiration.
  5. If indigestion of ? Hazardous substance; attempt to ID substance / label but do not bring container to hospital. Consider involvement of hazmat team.
  6. Charcoal is most effective if administered immediately after the ingestion. Contact poison control early and administer promptly if indicated.
  7. Anticipate vomiting complications during administration. Consider zofran.
