How do we know if a pregnancy is at risk?
ASSESSMENT- < Testing (pregnancy, CBC) > Date of LMP < General Health History > Potential abuse < Socioeconomic status (nutritional deficits, lack of prenatal care)
What is the most common disorders during pregnancy?
Anemia (iron or folic acid deficiency) Asthma (decreased lung volume) Cystic Fibrosis (increased secretions) Gallbladder (high fat diet) Pruritus (itching - liver not functioning well) Diabetes Hyper-/Hypothyroidism HIV (Epilepsy and Lupus - hard to conceive on these meds)
A C-Section is recommended if you have any history of a _______ disorder.
Cardiac
What patients are more at risk for pre-eclampsia?
Those with cardiac disorders
What is the 2nd leading cause of maternal morbidity/mortality in the U.S.?
Hypertension
What is gestational hypertension?
Increased BP, resolved after 6 weeks postpartum.
What is transient hypertension?
Increased BP after 12 weeks, resolved after 12 weeks postpartum.
What is HELLP syndrome?
Hemolysis Elevated Liver enzymes Low Platelet count
What are the types of miscarriages?
Threatened: slight spotting and slight cramping - bedrest
Inevitable: moderate bleeding and cramping - d/c
Incomplete: heavy bleeding and severe cramps - d/c
Missed: fetus had died w/out expulsion - d/c
d/c = dilate and clean out
What pill can cause a missed miscarriage or ectopic pregnancy to pass?
Methotrexate.
What is placenta previa?
Placenta implanted in lower uterine segment near or over the cervical opening.
What is hydatiform mole?
Abnormal proliferation and the degeneration of the trophoblastic villi - uterus increases in size but no embryo formation.
