Requisition Form Information
patient’s first and last name; ID #; patient’s sex; patient’s DOB; Patient’s location; name of ordering health-care provider; tests required; requested date and time of specimen collection; status of specimen (STAT, timed, routine); # and type of tubes; special collection info; special patient info; billing info; diagnosis codes
entering patient room
look for room signs, greet patient, ID patient
greeting the patient
introduce yourself, tell them why you are there in nontechnical terms, listen to and answer patient questions
time a tourniquet can be on
max 1 minute
needle insertion
15 to 30 degrees bevel up
tourniquet removal
remove after attaching final tube; CAN NOT be on patient when needle is removed
bandaging site
check for bleeding before applying bandage
tube labeling
must be labeled in presence of patient; if no printed label write patient’s name and ID #, date and time of collection, and phlebotomist initials on tube
vein of choice
median cubital then cephalic then basilic
