Communication
the exchange of information
The Sender
the person with information to share
-the sender delivers the information in the form of a message
The Receiver
the person for whom the information is intended
-through feedback, or a return message, the receiver lets the sender know whether the message was received and understood
Verbal Communication
a way of communicating that uses written or spoken language
Examples of verbal communication
- telephone conversations
- writing out a question to a deaf resident
- written notes and memos
- email and text messages
- ASL (American Sign Language)
Nonverbal Communication
a way of communicating that uses facial expressions, gestures, and body language, instead of written or spoken language
Examples of nonverbal communication
- making a face when you put weight on a painful leg
- nodding as someone speaks
- tapping your fingers on the table because you are bored
When to use a picture board?
when someone
- speaks a different language
- hearing impaired
- has a developmental disability
Conflict
discord resulting from differences between people, can occur when one person is unable to understand or accept another’s ideas or beliefs
Observation
something that you notice about the patient or resident, typically related to a change in the persons physical or mental condition
Objective Data
information that is obtained directly, through measurements or by using one of the five senses (sight, smell, taste, hearing, touch)
-“signs”
Objective Data examples
-elevated temperature
-a rash
-low urine output
These are called signs
Subjective Data
information that cannot be objectively measured or assessed
-“symptoms”
Subjective Data examples
-pain
-nausea
-dizziness
These are called “symptoms”
Medical Record
a legal document, also called a medical chart, where information about a patients current condition, the measures that have been taken by the medical and nursing staff to diagnose and treat the condition, and the patients response to the treatment and care provided is recorded
