Integrated care
an incident that did not reach the patient (no harm resulted)
Near miss
an incident that reached the patient, but no discernible harm resulted
No-harm incident
the degree to which health care services for individuals and populations increase the likelihood of desired health outcomes that are consistent with current professional knowledge
Quality of care
a physical, chemical, or environmental means of controlling an individual’s behaviour or actions
Restraint
