Chapter 18 - Special Tests Flashcards
Wallenberg’s Test - purpose
Describe
Test for vertebral artery insufficiency
Flex, extend, rotate, rotate + extend (hold for 10 sec)
Look for dizziness, visual changes, nystagmus, etc.
Underberg’s test - purpose
Describe
Test for vertebral artery insufficiency
Fully extend and rotate head, waiting for vascular or neurologic symptoms
Adson’s test - purpose
Scalenes
Wright’s test - purpose
Pec minor
Costoclavicular syndrome test - purpose
Clavicle-Rib
Apley scratch test - purpose
3 steps
Test glenohumoral ROM
- Reach behind head and tough shoulder (ABD, ER)
- Reach over and touch shoulder (ADD, IR)
- Reach behind back and touch scapula (ADD, IR)
Speed’s test - purpose
Explain
Bicipital tendon in bicipital groove
Hold arms out, flexed, supinated, w/ extended elbow
Resist shoulder flexion
Yergason’s test - purpose
Explain
Bicipital tendon in bicipital groove
Elbow at 90º, doc grabs elbow and wrist, doc pulls down on elbow and externally rotates shoulder. Pt resists.
Hip drop test - purpose?
What should you see? (2)
Lumbar and thoracolumbar sidebending
ConVEXITY on the dropped side
20º or more hip drop
Straight leg test - full test
Straighten leg until pain
Lower leg to just below pain, then DORSIFLEX foot
Pain w/ dorsiflexion = SCIATIC nerve problem
No pain w/ dorsiflexion = hamstring problem
Where do your thumbs go in the seated/standing flexion test?
Inferior notch below the PSIS
Pelvic side shift - purpose
Explain
Positive test?
Determine if sacrum is in midline
Stabilize shoulders, push pelvis each direction and look for asymmetry
POSITIVE = FREER translation
How exactly to do Ober’s test?
Bend knee, fully abduct hip, and SLIGHTLY EXTEND
Bounce Home Test - purpose
Explain
Positive test?
Test for full knee extension (meniscal tears, effusions, etc)
Fully flex knee, then allow to drop into extension. Should bounce to a sharp end-point
POSITIVE = incomplete extension or “rubbery” end
Apley’s Compression and Distraction - purposes
Compression - meniscal injury
Distraction - ligament injury (MCL, LCL)
