Chapter 18: Fetal assessment during labor Flashcards
Involves listening of the fetalheart sounds at periodic intervals to assess the FHR.
Internmittent auscultation
According to AWHON IA frequencies must be done:
every hour in the latent phase of 1st stage of labor.
every 15 minutes in the second stage
measures UA transabdominally, placed over the fundus above the umbilicus.
Tocotranducer (tocodynamometer) - have the woman lay in semi-fowler or lateral position
___ of the FHR can be described as irregular waves or fluctuations in the baseline FHR of two cycles per minute or greater
variability
changes in the FHR that occurs with UA
Periodic changes
changers in the FHR that does not occur with UA
Episodic changes
___ of the FHR is defines as a visually appatent abrupt increase in the FHR above the baseline rate
Accelerations (either periodic or episodic)
caused by dominance of a parasympathetic response may be benign or abnormal
decelerations
____ of the FHR is visually apparent gradual DECREASE in and return to baseline (>30 secs) associated with UA. Benign. Associated with fetal head compression. Does not need intervention.
Early decel
Uteroplacental insufficiency causes by?
late decel- concerning, can cause fetal hypoxemia, reposition mom, can lead to metabolic acedemia.
umbilical cord compression is cause by?
variable decel- reposition mom and amnioinfusion treatment
worst FHR of at least 15 bpm below the baseline and lasting more than 2 minutes but less than 10 minutes and can result in fetal hypoxemia.
prolonged decel- caused by disruption of fetal oxygen supply.
