Long-Term Care
Giving in long-term care facilities for people wo need 24-hr skills care
Skilled Care
medically necessary care given by a skilled nurse or therapists
Length of stay
the number of days a person stays in a care facility
Chronic
conditions that last a long period of time
diagnoses
medical conditions determined by a doctor
Home health care
care that is provided in a person’s home
Assisted living
facilities are resicendes for people who need some help with daily tasks
Adult day services
facilities for people who need some help and supervision during certain hours, but do not live in the facility
Acute care
24-hr skilled care given in hospitals and ambulatory surgical centers
Subacute care
care given in hospitals or long-term care facilities. Used for people who need less care than for an acute, but more care than for chronis
Outpatient care
care given to people who have had treatments, procedures or surgeries, and need short-term skilled care
Rehabilitation
care given by specialists.
Hospice care
care given in facilities for people who have about 6 months to live
Activities of Daily Living (ADL’s)
Personal care: bathing, skin, nail, hair, mouth care - walking, drinking, dressing, transferring, and elimination
Medicare
federal health insurance for people over 65 - also covers people with permanent kidney failure or certain disabilities
Medicaid
Medical assistance program for low income, and disabilities
Culture Change
process of transforming services for elders
Person-centered care
emphasizes the individuality of the person who needs care, and recognizes and develops his/her capabilities
Charting
the process of documenting daily activities and changes in residents
CNA - Certified Nursing Assistant
Performs assisgned tasks; vital signs and providing personal care for residents
RN- Registered Nurse
In long term care, coordinates, manages and provides skilled nursing care to residents
LPN/LVN - Licensed Practical Nurse/Licensed Vocational Nurse
A nurse who gives medications and treatments to residents
Physician/Doctor
Diagnoses desease or disability and prescribes treatment
Physical therapist
evaluates a person and develops treatment
Occupational Therapist
helps residents learn to adapt to disabilities using assistive/adaptive devices
Chain of command
the line of authority in a hospital or facility
Liability
legal term: someone can be held responsible for harming someone else
Scope of Practice
defines the tasks that heathcare providers are legally allowed to do
Care plan
individualized for each resident - helps to achieve goals of care
Policy
a course of action that should be taken

the method to achieve the policy

Abuse
purposeful mistreatment that casues physical, mental or physical, mental or emotional pain or injury to someone
Physical Abuse
any treatment (intentional or not) that casues harm to a person’s body
Psychological abuse
Emotional harm caused by threatening, scaring, humiliating, intimidating, isolating, or insulting a person, or by treating him like a child
Verbal abuse
use of spoken or written words, pictures, or gestures that threaten, embarras, or insult a person
Sexual Abuse
forcing of a person to perform or participate in sexual acts again their will
Financial abuse
improper or illegal use of a person’s money, possessions, property or other assets

a threat to harm a person, resulting in a person being fearful

intentional touching of a person without his/her consent
Domestic violence
abuse by spouses, intimate partners, or family members. Can be physical, sexual or emotional
False Imprisonment
unlawful restraint that affects a person’s freedom of movement
Involuntary seclusion
separation of a person from others against their will
Workplace violence
abuse of staff by other staff members, residents, or visitors
Sexual harassment
any unwelcome sexual advance or behavior that creates an intimidating, hostile or offensive working environment.
Substance Abuse
repeated use of legal/illegal drugs, cigarettes, or alcohol in any way that harms themselves or others
Neglect
failure to provide needed care that results in physical, mental or emotional harm to a person
Active neglect
purposeful failure to provide needed care, resulting in harm to a person
Passive neglect
unintentional failure to provide needed care, resulting in harm to person
Negligence
actures, or the failure to act or provide the property care for a resident, resulting in unintended insury
Malpractice
a person is injured due to professional misconduct through negligence, carelessness or lack of skill
Omnibudsman
assigned by law as legal advocate for residents
Confidentiality
keeping private things private
Health Insurance Protability and Accountability Act (HIPPA) (1996)
law to help keep health information private and secure
Protected health information (PHI)
Information that can be used to identify a person and relates to patient’s condition, any health care that the patience has had, and payment for health care
Minimum Data Set (MDS)
Detailed form with guidence for assessing residents - also list what to do if resident problems are identified
