What is the nursing process used for?
To provide appropriate delivery of patient care
What is the nursing process? (5)

In which phase does the patient problem belong in for the nursing process?
Diagnosis! Or analysis
What type of model do we want to use when providing patient focus care and why not the other one?
Patient centered model
Instead of
Disease centered model
Is it a more holistic view to give a patient a patient center model rather than a disease model?
Yes!
What does the term concept map or more so patient focus model mean or include?
Patient health, illness, health promotion
Preventative, acute and chronic health
What type of data is collected in the assessment portion of the nursing process? (2)
Objective and subjective
What is subjective data?
What the patient tells you!
( I’m in pain ! )
What is objective data?
What you can see on a patient !
( they have blue lips, cyanosis )
What is going on in the diagnosis phase of the nursing process?
Taking those objective and subjective data to come to a definitive conclusion
What is going on in the planning stage of the nursing process?
Identify the expected outcomes of the patients diagnosis and set intervention methods to meet those outcomes
When in the planning stage, are the interventions being made are specific or not? And why?
Yes
Because we wanted to see improvement, not cause harm!
How can you achieve a better patient centered outcome?
Include patient and family!
What is going on in the nursing intervention phase of nursing process?
Taking action in the care for the patient to accomplish the expected outcomes
Why is it important for the nurse to keep in mind factors that help promote patient learning?
Because we want to see if they are ready to learn!!!
Accommodate them ( language barrier )
Is pain an obstacle in patient care? Why? And how can we teach our patient when in pain?
Yes it is
Because it hurts and all we focus on is being hurt
We don’t teach or postpone until pain is relieved
When teaching patients to self administer or informing them on diet, medications and side effects, what method do we want to use?
Teach back!
( repeat it back to me or show me how to do it )
What is the evaluation phase of the nursing process?
Seeing if the patient has met the outcomes ( better )
If the patient didn’t met the outcomes, what do we do?
We reassess, plan, intervene and evaluate
What is the clinical judgment model? (6)
Recognize cues
Analyze cues
Prioritize hypothesis
Generate solutions
Take actions
