General Coding Guidelines for Diagnosis
Acute
A short and relatively severe course
Chronic
Persistent over a long period
Combination code
A single code used to classify two diagnoses; or a diagnoses with an associated secondary process (manifestation); or a diagnoses with an associated complication
Conventions
General rules for use in classification that must be followed for accurate coding
Etiology
Cause or origin of a disease or condition
Manifestation
Symptom or condition that is the result of a disease
Residual condition or effect
When the acute phase of an illness or injury has passed, but a residual condition or health problem remains
Sequelae
Residual conditions or effects; time when the acute phase of an illness or injury has passed, but residual conditions or health problems remain
Sign
Objective evidence of a disease or of a patient’s condition as perceived by the patient’s examining physician
Subacute
Somewhat acute; between chronic and acute
Symptom
Subjective evidence of a disease or of a patient’s condition as perceived by the patient
