What are the most concerning effects of using opioids?

For someone with pain, what do we want to make sure we identify?

What is best practice when administering opioids (i.e. how do we want to administer them)?
Start slow, go slow
What do we want to try before we get to opioids?
Try nonopioids (acetaminophen) or nonsteroidal anti-inflammatory drugs (ibuprofen)
