What are the indications for central venous catheterization
A
- Emergency venous access (failed peripheral venous access)
- Administration of irritant substances
- Vasoactive agents
- Chemotherapy
- TPN - CVP monitoring
- ScvO2 monitoring/sampling
- Renal Raplacement Therapy
- Plasmapharesis/apharesis
- Transvenous pacing

A
Rapid infusion catheter
- Guidewire and dilator - sited via existing cannula

Are CVL preferred for rapid fluid resuscitation
A
No
- Large bore peripheral IV
- RICC lines
- IO

What are contraindications to CVP insertion
A
- Coagulopathy
- Respiratory Failure
- Raised ICP (cannot tilt head down)
CAN USE FEMORAL APPROACH IN ALL ABOVE SITUATIONS
- Obstructed vein (thrombus/tumour)
- Overlying skin infection/burn or other disease process
- Haemorrhage from target vessel
- Unco-operative patient

- Pneumothorax highest for supraclavicular approach)
- Failure to locate vein
- Accidental arterial puncture
- Haemothorax
- Haematoma
- Arrythmia
- Thoracic duct injury
- Guide wire embolus
- Air embolus
Early
- Haemopericardium and tamponade
- Pneumothorax
- Catheter blockage
- Chylothorax
- Catheter knots
Late
- Infection (no difference of incidence regardless of site: IJ/SC/Femoral)
- Catheter fracture
- Vascular erosion
- Vessel stenosis
- Thrombosis
- Osteomyelitis of clavicle
