Ceftriaxone (Rocephin)
classification, method of action, indications
generic name: ceftriaxone
brand name: Rocephin
classification: antibiotic-cephalosporins 3rd generation
method of action: inhibiting the mucopeptide synthesis in the bacterial cell wall.
indications: lower respiratory tract infections, acute bacterial otitis media, skin and skin structure infections, UTIs, uncomplicated gonorrhea, pelvic inflammatory disease, bacterial septicemia, bone and joint infections, intra-abdominal infections, meningitis, and also can be used for surgical prophylaxis.

common side effects?
mild diarrhea,warmth, tight feeling, or a hard lump where the injection was given, vaginal itching or discharge, rash, and
abnormal liver function test
more side effects:
Hypersensitivity: Maculopapular rash, Urticaria, Pruritis, eosinophilia
Hematologic: Hypoprothrombinemia, Neutropenia, Leukopenia, Thrombocytopenia
GI: Diarrhea, C. difficile disease
Renal: Interstitial nephritis
life threatening side effects?
larngyoedema from allergic reactions, anaphylaxis/angioedema
pregnancy classification: unknown since no data, safe in animals
antidote: n/a, just dc it
FDI: NO USING with calcium containing solutions–precipitates. not recommended with a live cholera vaccine, not recommended with warfarin.
lab tests:
Toxic: Urinalysis, BUN, SCr, AST and ALT, skin rash, neutropenia and leukopenia, Prothrombin time in patients with renal or hepatic impairment or poor nutritional state, as well as patients receiving a protracted course of antimicrobial therapy, and patients previously stabilized on anticoagulant therapy.
nursing assessment pre admin:
assess for penicillin allergy
nursing assessment post admin:
assess for allergic reactions and watch labs.
method of administration: IV or IM
*****DO NOT use diluents containing calcium, such as Ringer’s solution or Hartmann’s solution, to reconstitute Rocephin vials or to further dilute a reconstituted vial for IV administration because a precipitate can form.
patient/family education: Drink plenty of liquids to keep your kidneys working properly while using ceftriaxone. Use this medicine for the full prescribed length of time, even if your symptoms quickly improve. Skipping doses can increase your risk of infection that is resistant to medication.
