brand name: Maxipime
classification: fourth generation cephalosporins
method of action: Cefepime inhibits bacterial cell wall synthesis by covalently binding enzymes responsible for the final step in transpeptidation during peptidoglycan wall synthesis. This binding causes defects in the cell wall leading to autolysis and subsequent death of the organism.
indications: Uncomplicated skin and skin structure infections, Bone and joint infections, Uncomplicated and complicated urinary tract infections, Respiratory tract infections, Complicated intra-abdominal infections (with metronidazole), Septicemia. Empiric treatment of febrile neutropenic patients.
pregnancy classification: no data but it does cross placenta so would be best to find a better drug

FDI: Drug-Drug: Probenecid ↓ excretion and ↑ blood levels. Concurrent use of loop diuretics or aminoglycosides may ↑ risk of nephrotoxicity. NO food interactions
lab tests:
Therapeutic: Culture and sensitivities, serum levels, signs and symptoms of infection, white blood cell count
Toxic: Urinalysis, BUN, SCr, AST and ALT, skin rash, Neutropenia and leukopenia,prothrombin time in patients with renal or hepatic impairment or poor nutritional state, as well as
patients receiving a protracted course of antimicrobial therapy, and patients previously stabilized
on anticoagulant therapy.

vitals, penicillin allergy, labs, etc.
nursing assessment post admin:
allergic reaction, side effects, etc.
method of administration: IM or IV
patient and family education: Advise patient to report signs of superinfection (furry overgrowth on the tongue, vaginal itching or discharge, loose or foul-smelling stools) and allergy.Instruct patient to notify health care professional if fever and diarrhea develop, especially if stool contains blood, pus, or mucus. Advise patient not to treat diarrhea without consulting health care professional.

Contraindicated in: Hypersensitivity to cephalosporins; Serious hypersensitivity to penicillins.
Use Cautiously in: Renal impairment (↓ dosing/↑ dosing interval recommended if CCr ≤60 mL/min); History of GI disease, especially colitis; Patients with hepatic dysfunction or poor nutritional status (may be at ↑ risk of bleeding); Geriatric patients (dose adjustment due to age-related ↓ in renal function may be necessary); OB, Lactation: Pregnancy and lactation (safety not established).
common side effects: nausea, vomiting, diarrhea; fever; headache;
itching, rash; or abnormal blood tests
life threatening side effects: anaphylaxis from allergic reactions
