classification: cephalosporin derivatives
method of action: ceftaroline’s mechanism of action is mediated by binding to the penicillin-binding protein (PBP), the enzyme mediating the cross-linking transpeptidation of the peptidoglycan which are the terminal steps in completing formation of the bacterial cell wall.

indications: Acute bacterial skin/skin structure infections. Community-acquired pneumonia.

Cefatroline side effects, contraindications/precautions, labs

A

common side effects: Derm: rash. GI: CLOSTRIDIUM DIFFICILE-ASSOCIATED DIARRHEA (CDAD), diarrhea, nausea. Hemat: hemolytic anemia. Local: phlebitis at injection site. Misc: HYPERSENSITIVITY REACTIONS (including anaphylaxis).

larngyoedema d/t anaphylaxis

contraindications and precautions:
CONTRAINDICATED in: Known serious hypersensitivity to cephalosporins.
CAUTION in: Known hypersensitivity to other beta-lactams; Renal impairment (dosage ↓ required for CCr ≤ 50 mL/min in adults); OB: Use only if potential maternal benefit justifies potential fetal risk; Lactation: Use while breast feeding only if potential maternal benefit justifies potential risk to infant; Pedi: Neonates <34 wk (gestational age) or <12 days (postnatal age) (acute bacterial skin/skin structure infections) and infants <2 mo (community-acquired pneumonia) (safety and effectiveness not established); Geri: Dose adjustment may be necessary for age-related ↓ in renal function.

nursing assessment post admin:
Observe patient for signs and symptoms of anaphylaxis (rash, pruritus, laryngeal edema, wheezing). Discontinue the drug and notify health care professional immediately if these symptoms occur. Keep epinephrine, an antihistamine, and resuscitation equipment close by in the event of an anaphylactic reaction.
Monitor bowel function. Diarrhea, abdominal cramping, fever, and bloody stools should be reported to health care professional promptly as a sign of Clostridium difficile-associated diarrhea (CDAD). May begin up to several mo following cessation of therapy.

method of administration: IV

patient and family education: Explain the purpose of ceftaroline to patient. Emphasize the importance of completing therapy, even if feeling better.Instruct patient to notify health care professional if fever and diarrhea develop, especially if stool contains blood, pus, or mucus. Advise patient not to treat diarrhea without consulting health care professional.
