HACEK organisms?
Haemophilus, Actinobacillus, CArdiobacterium, Eikenella, Kingella. Likely culture negative
NAtive Valve Enterococcus tx?
Penicillin and gentamicin
Native valve Bartonella tx?
Doxycycline and gentamicin
Native Valve strep Pyogenes?
Pen G or Rocephin
Prosthetic valve Staph tx?
Rifampin, Gent, staph coverage based on culture
Prosthetic Valve strep viridans tx?
Pen G or Rocephin and if resistance add Gentamicin
Prosthetic valve enterococcus tx?
Ampicillin plus gent
Prosthetic Valve VRE tx?
Linezolid
Prosthetic Valve HACEK tx?
Rocephin or Unasyn or Cipro
how long to hold AC after CNS emboli with prosthetic valve?
2 weeks
complications of infected device removal?
hemothorax, tamponade
