Exercise->limited by SOB, palpitations, chest pain.
Colour change->cyanosis, pale, sweaty, poor perfusion
Growth->feeding problems, poor weight gain
Syncope->collapse w/ exercise, unexplained, palpitations
Murmurs->previously noted murmur
Family history->CHD, sudden death in young people
