Q

What are the different types of ANGINA ?

A
  1. ) STABLE
    • Happens at predictable times ( exercise / stress ).
  2. ) UNSTABLE
    • Happens when no particular demand is being placed on the heart.
  3. ) VARIANT
    • When a coronary artery goes into spasm.
  4. ) MICROVASCULAR
    • Affects the very smallest vessels.

Q

What is MYOCARDIAL INFARCTION ?

A

Its when the blood supplying the O2 to the heart is severely reduced or cut off.

( This results in Ischaemia of the heart muscle and scar formation )

Q

What is HEART FAILURE ?

A

Its when the heart is unable to adequately pump blood around the body usually because the heart has become stiff / weak.

Q

What happens to the valves of someone with a Heart Valve Disease.

A
  1. ) VALVES NOT FULLY OPEN
    - restricting blood flow = stenosis ( narrowing )
  2. ) VALVES DOESNT CLOSE PROPERLY
    - blood flows backwards.

Q

What are the different types of HEART VALVE REPLACEMENT ?

A
  1. ) MECHANICAL ( pyrolytic carbon )
    - Its long lasting 15-25 y
    - Patient requires long term anticoagulants medication
  2. ) TISSUE ( bioprosthetic )
    - It’ll last 10-20 y
    - It comes from an animal donor
  3. ) DONOR
    - last about 10-20 y
  4. ) ROSS PROCEDURE
    - Aortic valve is replaced with pulmonary valve which is subsequently replaced.

15
Q

How is the Valve Replacement Procedure done ?

A
  1. ) General Anaesthesia
  2. ) Incision to the heart is made, allowing surgeons to see the valve.
  3. ) Diseased valve is then removed and the size for the new valve is determined.

4.) Once the size has been determined, valve is stitched around the annulus and seated
into place.

5.) SALINE is then flushed through the valve to check it’s competency.

6.) Heart is closed up, patient is taken off the Heart & Lung machine to restore normal
function.

Q

What is Coronary Artery Bypass Graft (CABG) ?

A

Its a surgical procedure used to treat CHD, involves bypassing a blocked portion of a coronary artery using another piece of blood vessels.

17
Q

What are the Complications that can arouse after Heart Valve Replacement ?

A
  • STROKE
  • CLOTTING
  • ARRYTHMIA
  • CHEST INFECTION
  • PLEURAL EFFUSION
  • KELOID SCARRING
  • WOUND INFECTION
  • NON UNION STERNUM
18
Q

Describe the Procedure for CABG.

A

1.) GA, Incision is made via MEDIAN STERNOTOMY.

2.) The heart is stopped during surgery and kept going via Cardio Pulmonary bypass.
(CPB)

  1. ) The blocked portion of the coronary artery is bypassed with another piece of blood
    vessel.

4.) One end of the graft is attached above and below the blockage, thus bypassing it to
reach the myocardial tissue.

19
Q

What are the possible complications of a CABG Surgery?

A
  • LOW CARDIAC OUTPUT
  • GRAFTS ONLY LAST 8 - 15 y
  • LATE GRAFT STENOSIS
  • PULMONARY OEDEMA
  • RENAL IMPAIRMENT
  • CHEST INFECTION
  • KELOID SCARRING
  • HAEMOTHORAX
20
Q

What is Transplantation ( in cardiac surgery ) ?

A

It has to be from a beating donor, its normally needed in severe CHD.

21
Q

What are some possible Transplantation Complications ?

A
  1. ) REJECTION
    - immune system recognising the transplant heart as foreign therefore attacking it.
  2. ) GRAFT FAILURE
    - donated heart could fail to work properly.
  3. ) CARDIAC ALLOGRAFT VASCULOPATHY
    - narrowing of the arteries supplying the heart.
  4. ) SIDE EFFECTS FROM IMMUNOSUPPRESSANT MEDICATION.
    - increased infection vulnerability / weight gain
