What is the most common type of cardiomyopathy that can cause HF?
Dilated cardiomyopathy
What is happening to the heart in dilated cardiomyopathy?
Muscles of ventricles thickened and ballooned out causing thin walls and reduced CO.
(weak contractions, thinned walls, normal filling)
What are some clinical manifestations of dilated cardiomyopathy?
- -L/R sided HF
- -S3 and S4 gallop
- -Orthopnea
- -Paroxysmal Nocturnal dyspnea
- -JVD
What type of thrombi is common in dilated cardiomyopathy?
mural thrombi
What can cause dilated cardiomyopathy?
- -alcoholism
- -myocarditis
What can cause hypertrophic cardiomyopathy?
Chronic HTN
What is going on in Hypertrophic cardiomyopathy (HCM)?
The ventricle walls thicken, Which restricts filling (reduces preload), then reduces CO
(thick walls, small ventricle area for filling)
What are some clinical manifestations of HCM?
- -Loud S4
- -angina
- -Syncope
What meds should be avoided in a person with HCM that decrease preload?
Nitrates, diuretics, morphine
What meds should be avoided in a person that has HCM that increase preload?
Dopamine, Digoxin, Isoproterenol
Describe Restrictive cardiomyopathy?
When the right ventricle is rigid causing a problem with filling. This leads to reduced SV and CO.
What are some manifestations of RCM?
- -RSHF
- -JVD
- -S3 and S4 gallop
- -mitral regurgitation murmur
What type of treatment is not a viable option for RCM?
Transplantation
What are some ways to improve CO in a patient with cardiomyopathy?
- -Rest
- -Position legs down
- -O2
