Primary Cardiomyopathies
due to an idiopathic process effecting the myocardium
Secondary Cardiomyopathies
related to a specific heart muscle disease
Dilatated (Congestive, DCM, IDC)
ventricular enlargement and systolic dysfunction, from ischemic heart disease
Hypertrophic (IHSS, HCM, HOCM)
inappropriate myocardial hypertrophyin the absence of HTN or aortic stenosis… diastolic dysfunction
Restrictive (Infiltrative)
abnormal filling and diastolic function, think post radiation and amyloid cases
IDC
a disease of unknown etiology that principally affects the myocardium
LV dilatation and systolic dysfunction
Alcohol septal ablation (NSRT)
Controlled myocardial infarction of the basal ventricular septum to ↓ gradient.
First septal artery occluded with a balloon catheter and ETOH injected distally
Restrictive CardiomyopathyDiagnosis
**CXR-signs of CHF without cardiomegaly
EKG-nonspecific changes most likely
ECHO-thickened LV walls with normal or slightly decreased volume
Amyloid heart disease
Thickened, firm, rubbery ventricular muscle
Major diastolic dysfunction
Abnormal Q waves – look like an M.I.
Conduction disturbances, arrhythmias not uncommon
Echo: thickened walls everywhere
