Long QT 1 syndrome: precipitants?
adrenergic surge due to intense physical activity such as swimming
Long QT 2 syndrome: precipitants?
adrenergic surge due to intense emotion such as excitement or fear
Long QT 3 syndrome: usually presents as?
Death during sleep
2nd line of refractory VT after amiodarone?
Intravenous lidocaine: first as a bolus, followed by an infusion regime over 6.5 hours.
** use with caution in severe left ventricular impairment
Management of symptomatic sick sinus syndrome?
AAIR pacemaker:
- sense and pace the atria
- for isolated sinoatrial node disease
- NOT suitable in atrial fibrillation
Or
DDDR pacemaker
Recommended pacemaker setting in pure slow AF?
VVI or VVR
- no need to pace atria as they are not functioning
Pacemaker for mobitz type II block?
DDD or DDDR
> DDD = record both atrial and ventricular rates and pace either chamber
> DDDR is the same but can adjust the heart rate to account for the need for higher cardiac output.
When to offer PCI in stable angina?
ESC guidelines recommend offering revascularisation to patients with stable coronary artery disease and ischaemia in > 10 % of the left ventricle
