Blood cultures for IC
Taken and can be incubated aerobically or anaerobically for 7 days
Blood cultures should be taken away from site on infection why?
Avoid contamination of sample - see if you really have systemic sepsis
What will optochine disc do?
- Diff between strep pneumo and Beta haemolytic strep
- Strep pneumoniae - optochine sensitive
- Others are optochine resistive
Tx for strep viridians in IC
IV Benzyplenicillin for 4-6 weeks plus or minus gentiamicin
Main cause of IC
SA due to IVDU
Tx = flucocaxiclin - vancomycin if MRSA
Septic emboli can cause what?
- Complications due to vegetations
- Vegetations - thrombus which occur in chamber of heart
- bacteria use thrombus to feed on and grow - fragment of and cause septic emboli that lodge in diff vessels
Staph epidermis can also cause IC - Features
In immunocompromised
Streptococci classification - lancefield - Beta haemolytic
A : Pyogenes - pharyngitis, cellulitis, glomerulonephritis
B: Agalacticae - neonatal meningitis + septicaemia
C: pharyngitis
D: Cellulitis
Streptococci classification - lancefield - Alpha- haemolytic
Viridians group - dental abcesses, IE
S. pneumoniae - otitis meida, sinusitis, pmenumonia
