Explain what happens in PCI (aka Angioplasty with stent)
(Smaller procedure compared to CABG)
- Get access through femoral/ radial/ brachial artery
- Pass guide wire into relevant artery
- Dilate with balloon catheter
- Place stent
Explain what happens in CABG
(Major surgery)
- Make mid-sternal incision
- Graft blood vessel eg saphenous vein/ internal mammary artery
- Place blood vessel around blocked area to create alternate path for blood to pass through
Explain what happens in thrombolysis for MI
Thrombolytic drug (streptokinase/ alteplase) +
Anticoagulant
heparin/ LMWH/ fondaparineux
Main PCI indications
ACS in patients with less complex vessel disease
Patients you want to avoid surgery in
Main CABG indications
ACS in patients with complex vessel disease (L main disease/ multi vessel disease/ LV disease
ACS in diabetics
Main thrombolysis indications
ACS but unable to do PCI/CABG within 90min
Main PCI complications
- Risk of needing repeated tx
- Stent thrombosis (MI)
- Stent stenosis (return of angina)
Main CABG complications
Stroke
Main thrombolysis complications
- Risk of re-occlusion (not as ideal as the other interventions)
- Bleeding risk eg ICH
What drugs should be given to someone going for PCI
Heparin OR
LMWH
