ACE inhibitors (ends with PRIL) and ARBs (ends in SARTAN)
May cause nagging dry cough
Arterial dilation and increased stroke volume
Prevents Na and H2O retention, decreasing FVO
ACE inhibitors nursing considerations
Monitor for hypotension especially after the first dose
ACEI can cause angioedema (swelling of tongue and throat), decreased sense of taste or skin rash
Monitor for hyperkalemia- adverse effect especially if they have renal problems
Angiotensin receptor neprilysin inhibitor (ARNI)
Used in place of ACEI or ARBs and should not to be given to the pt with history of angioedema
Side effects of ARNI
Hypotension
Hyperkalemia
Cough
Dizziness
Renal failure
Monitor potassium and creatinine levels
Human B type Natriuretic Peptide
Causes loss of sodium in renal tubules and vasodilation
Interventions that reduce preload
Reduce Sodium intake 2g daily
High sodium (bacon, ham, pickles, table salt)
Some pts are placed on strict fluid restriction
Weigh pt daily
Diuretics
Reduce preload
Helps with HF and fluid overload
Reducing systemic and pulmonary congestion
Morphine
Reduce anxiety, decrease, preload and afterload, slow, respirations, and reduce pain associated with MI
