Cardiac Biomarkers Flashcards
What 3 dz’s are part of acute coronary syndrome?
Unstable angina
NSTEMI
STEMI
What makes angina unstable?
New
occurs at rest
lasts longer
ECG may show ST depression or T-wave inversion/flattening
What is the universal definition of MI?
a rise in cardiac biomarkers
+
Evidence of ischemia
EKG changes of acute ischemia
- New Q waves
- Imaging evidence of new loss of viable myocardium or new wall motion abnormality
- Identification of coronary thrombus by angiography
What is the initial workup of ACS?
Cardiac Enz CBC PT and PTT CMP ECG
TIMI and GRACE scores predict what time frame of revent/death
TIMI - 14-day risk assessment
GRACE -6 months
What populations usually present with atypical MI sx?
Females
diabetics
elderly
What is the prefered cardiac biomarker to detect cardiac tissue necrosis?
Troponin
cTn-1 and cTn-T
When do you order Troponin?
at presentation and 6 hrs after onset of sx
