What occurs during cardiogenic pulmonary edema?

A

It’s when there’s a result in increase of hydrostatic pressure within the pulmonary capillary bed as a result of HEART FAILURE.
Causes could be: HF, MI, MR, Tamponade, dysrhythmia, HTN crsis

How do you differentiate between ARDS and cardiac pulmonary edema?

A

In cardiac there more silhouette to heart, fluid at bases of lung and in lung. Xray may show spider vein lines. The fluid is in the vessels of lungs.
In ARDS, fluid still in alveoli.

If you give lasix to both these pt, the cardiac pulmonary edema x ray would improve. NOT in ARDS because fluid is in alveoli which we can’t remove.

A
  1. diuretics
  2. analgesic
  3. preload reduction (nitro)
  4. O2
  5. afterload reduction if sBP >100 mmHg
  6. Vasoconstriction if hypotensive

A
  • tx underlying cause
  • block specific mechanism of alveolar capillary
  • minimize consequences of of ALI
