Change in FPCON,  threat or disturbance, Suspected security incident, loading, offloading, or transfer of weapons or pyrotechnics.

Casualty of the ship .
Collision fire, flooding, loss of power or damage of any kind .
Fuel oil diving operations, man aloft or hot work.
When to contact CO Operations?
Any time to get underway shift, Bert or change operational status?
Provide rescue assistance to another vessel or short facility .
Anytime to deploy the small boat
Pollution sighted or created
Severe storm warnings sustain winds of 35 kts or greater
Any media representative questions or desires to contact an interview .
When operating in near areas of restrictive visibility? When to Call CO?
- Reduce speed w/ nav rules.
- Energize nav lights, sound signals and fog signal.
- AIS switch.
- Low vis bill.
- Rule 19.
Visibility reduced to two nautical miles.
When operating darken ship operations?
When we assume the role of giveaway vessel remain clear all vessels with the CPA of two nautical miles 
Went to contact the CO via LE contacts?
Only approach a vessel within 1 nm or less with my permission.
Calling the captain via anchoring
Dragging anchor
Any vessel in our vicinity, which appears to be dragging anchor
Any vessel coming in to our anchorage or vicinity to weigh anchor.
Any vessel with the CPA of 250 yards by day or 500 yards by night
Wind exceeding 25 kn increases to 4 feet .
When to call the CO UW?
Watchstanding:
a. Cannot comply with night, standing, navigation rules or CG regulations.
b. No night orders by 2200.
c. cannot assume the watch.
d. uncertain.
Navigation:
a. doubt about position.
b. exceeding xtrack in coastal and open waters.
c. CMG is more than 10 degrees from ordered courst.
d. water depth diff from charted debth.
e. Loss of nav equipment
f. offsta aid.
g. when approved route shall be altered for any reason.
Environment:
a. Visibility can drop below 2 nm, confirm when opens back up.
b. Wind speed increase 10 kts or more in one hr, sustained true wind of 30 kts.
c. barometric change of 0.04 in one hr.
d. wave height increase by 2ft.
e. unusual or unexpected wx phenomenon.
Personal and equipment:
a. emer sound on bridge
b. accident or injury to personnel.
c. When starting and stopping MDE’s if not outlines in night or day orders.
Contact:
a. other vessel fog signal detected.
b. pop up contact 3nm.
c. Vessel has a CPA of less than 1nm by day and 2 NM by night and is less than 30 min CPA.
d. sighting of another Military vessel or CGC.
Mission:
a. Distress radio or message traffic
b. Spotting a TOI
c. Prior to launching and recovering small boat.
d. OPCON and TACON change.
Reasons to call EPO UW
- IAW EPO standing orders.
- ENG instructions can’t be met.
- Casualty to machinery or electronics.
- Inquiry to personnel in eng dept/space.
- abnormal readings
- Complete/partial power failure
- fire or flooding
- Prior to conducting work-opening strainer.
- Safety system override or bypass.
- Prior to working with flammable liquid line.
- Prior to hot work.
- Prior to work done by Mat, WAT and ESD..
- Prior to fluid transfer
- Any tank below 70% fuel or PW.
- any change in ship status
- Sewage GW, fuel reach 50%.

Darken ship OPS?
When operating LE, assume role as give way.
If CPA is less that 2NM call me with a plan to increase CPA.

M - metallic noise
P - personal entanglement
B - bravo fire
C - Charlie fire
