CA channel blockers
VERY NICE DRUGS!
V-verapamil
N- Nifedipine
D- Diltiazem
- works by inhibiting influx of Ca into cells, preventing muscular contraction -> vasodilation occurs
- decrease contraction of HR , decreased HR, decreased conduction
Similar to what other drugs……
Beta Blockers!!
Calcium channel blockers “dipines” Action
- work on primarily blood vessels
- vasodilation of arterioles and arteries
Prototype: dipine
Nifedipine (Procardia)
Nifidipine SE
flushing, dizziness, peripheral edema, postural hypotension, eczematous rash, gingival hypertrophy
Reflex Tachycardia: give BB to decrease SE
CA channel blockers non- dipines
Verapamil & diltiazem
- Act on arterioles and heart
- dec contractility (neg inotrope)
- dec HR (neg chronotrope)
- Dec conduction through AV node (neg dromotrope)
- Vasodilation and increased coronary perfusion
Verapamil (Calan)
- given orally or IV
- SE: constipation, dizziness, facial flushing, edema, in ankles/feet
- Can cause bradycardia, heart block and HF
Calcium Channel Blockers : Nursing Considerations
-Check BP and pulse (verapamil & diltiazem)
- Watch for reflex tachy (nifidipine)
- IV admin-tachy and hypotension
Don’t give w/ grapefruit juice
