What is ODD characterized by?
persistent patterns of angry mood and defiant behavior that occurs more frequently than is usually observed in individuals of comparable age and developmental level and interferes with social, educational, occupational or other important areas of functioning
what are the family influences that predispose a child to ODD?
power-struggle between child and parent for control with family dynamics that are similar to: a strong-willed child with reactive and high-energy temperament whose parents are authoritarian rather than authoritative, and the strong-willed child reacts to excessive parent control with anger and increased self-assertion
What is the diagnostic criteria for ODD?
has at least four symptoms lasting for at least 6 months
ANGRY/IRRITABLE MOOD: often loses temper, often touchy or easily annoyed, often angry and resentful
ARGUMENTATIVE/DEFIAN BEHAVIOR: argues with authority figures, actively defies or refuses to comply with requests form authority figures, often deliberately annoys others, often blames others for his/her mistakes or misbehavior
VINDICTIVENESS: has been spiteful or vindictive at least twice within 6 months
*mild: symptoms confined to only one setting
*moderate: symptoms persistent in at least two settings
*severe: some symptoms present in three or more settings
CP for noncompliance with therapy and nursing interventions for child with ODD.
STG: patient participates in and cooperates during therapeutic activities
LTG: patient completes assigned tasks willingly and independently or within a minimum of assistance
INTERVENTIONS: structured plan of therapeutic activities and start with minimum expectations and increase as the patient begins to manifest evidence of compliance, establish rewards for compliance in therapy and consequences for noncompliance, convey acceptance of the patient separate from the undesirable behaviors that are exhibited (not you but your behavior)
CP for defensive coping and nursing interventions of a child with ODD
STG: patient verbalized personal responsibility for difficulties experienced in interpersonal relationships within a time period reasonable for the patient
LTG: patient accepts responsibility for own behaviors and interacts with others without becoming defensive
INTERVENTIONS: help patient to recognize that feeling of inadequacy provide defensive behaviors such as blaming others for their problems or getting even, provide immediate nonthreatening feedback for passive-agressive behavior, helps identify situations the provoke defensiveness and practice through role-play, provide immediate positive feedback for accepted behaviors
CP for low self-esteem and the nursing interventions.
STG: patient partipates in own self-care and successes with the nurse aspect of self about which he/she feels good
LTG: patient demonstrates increased feeling of self-worth by verbalizing positive statements about self and exhibits fewer manipulative behaviors
INTERVENTIONS: ensure realistic goals, plan activities that provide opportunities for success, convey unconditional acceptance and positive regard, set limits on manipulative behavior, help the patient understand that he or she uses this behavior to try to increase own self-esteem
CP for impaired social interaction and nursing interventions.
STG: patient interacts with age-appropriate manner with nurse in one-to-one relationship within one week
LTG: patient is able to interact with staff and peers using age-approriate acceptable behaviors
INTERVENTIONS: develop a trusting relationship and convey acceptance of the person separate from the unacceptable behavior, explain tot he patient about passive-aggressive behavior and what is acceptable, provide peer group situations for the patient
