A

The IQ level to determine what level of IDD and focus on the level of independence for self-care the child is able to perform which will help to develop a nursing plan of care.

A

short and long-term goal exemplifies that the patient does not experience injury.
Nursing Interventions: create a safe environment, remove sharp objects, store patient items used frequently within reach, pad side rails and headboard of patient if HX of seizure activity, prevent physical aggression and acting out behaviors by learning to recognize signs that the patient is agitated

Short-term goal: patient is able to PARTICIPATE in aspects of self care
INTERVENTIONS: identify capabilities of patient, work on one aspect of self-care at a time, provide the patient with simple explanations offering positive feedback for efforts (enhances self-esteem)
Long-term goal: patient has all self-care needs met
INTERVENTIONS: when one aspect of self-care is mastered, move on to another, encourage independence but intervene when patient is unable to perform (RAT patient comfort and safety are using priorities)

A

Short-term goal: patient establishes trust with caregiver and a means of communication.
INTERVENTIONS: maintains consistency of staff to facilitate trust
Long-term goal: patients needs being met through established means of communication or needs are anticipated by caregiver if patient cannot communicate
INTERVENTIONS: anticipation of patients needs, determine special words patient uses, identify nonverbal gestures or signals patient may convey for communication, practice communication skills regularly (systemic habit training)

INTERVENTIONS: remain with the patient during initial interactions with other on the unit (feeling of security)
Long-term goal: patient is able to interact with others using behavior that are socially acceptable and appropriate to the patients developmental level
INTERVENTIONS: explain patients nonverbal gestures and signals to other patients, use simple language to explain which behaviors are acceptable and which are not, establish rewards for appropriate behaviors and adverse reinforcements for inappropriate behaviors (positive, negative, and adverse reinforcements contribute to desired changes in behavior)
