Common causes of viral bronchitis?
90% of cases viral. Influenza A & B, parainfluenza, RSV, coronavirus, adenovirus, rhinovirus
Common causes of bacterial bronchitis?
10% of cases. Bordetella pertussis, chlamydophila pneumonia, mycoplasma pneumonia
Commonly spread between family members?
Mycoplasma pneumonia
Causes a cough lasting longer than 4 weeks. Could have presence of inspiratory whooping heard?
Bordetella pertussis
Common causes of “common cold”
Rhinovirus and coronavirus
Frequently indicated in crowded conditions, military recruits
Adenovirus
Common symptoms/signs of bronchitis?
Appears as common cold, cough that last longer than 5 days, wheezes can be present, rhonchi can be present
What signs are uncommon in bronchitis?
Fever, systemic symptoms, toxic appearance, chills, sweating, myalgias, fatigue
When would you order imaging?
When pneumonia is suspected. RR >24, HR >100, Temp > 38 or 100.4
When would you order rapid PCR testing?
Pertussis and influenza suspected
Treatment for bronchitis caused by mycoplasma pneumoniae or chlamydophila pneumonia
Macrolide or fluoroquinolone
Azithromycin 500mg day 1, then 250mg day 2-5, doxycycline 100mg BID 7-14 days, or Levofloxacin 500mg QD 7-14 days
Treatment for bronchitis caused by bordetella pertussis?
Macrolides: Azithromycin 500mg day 1, then 250mg days 2-5, Clarithromycin 500mg x 7 days. Cant take macrolides: Bactrim DS 1 BID x 14 days
Common side affect of Clarithromycin?
Metallic taste
Upper airway cough syndrome causes?
Post nasal drip from respiratory infection, allergies, rhinitis, sinusitis. Cobblestone appearance in posterior throat
Typical asthma vs cough variant asthma?
Typical: wheezing, cough, dyspnea. Cough variant asthma: primary manifestation is asthma
