BPPV Flashcards
Conditions of the INNER EAR where the patients present ONLY WITH VERTIGO
BPPV - most common cause of vertigo
Vestibular neuritis
Typical history given by patients with BPPV
Vertigo lasting for only a few seconds upon POSITION CHANGE
Otolith is composed of what?
Calcium carbonate crystals
Most common scc involved in BPPV is
Posterior scc
Second m/c is lateral or horizontal scc
SUPERIOR SCC is almost NEVER INVOLVED
Treatment of BPPV
Repositioning the otolith and this manoeuvre of repositioning is different for each scc ; due to this it is important to identify the type of scc involved
How is the involvement of posterior scc and horizontal scc is identified ?
Posterior scc - Dix hallpike manoeuvre
Horizontal scc - supine roll
Vertigo is associated with nystagmus
What is ewald’s first law ?
It states that the direction of nystagmus is in the plane of the semicircular canal that is stimulated
Direction of nystagmus?
Direction of nystagmus is towards the more active semi circular canal, here the one that has the otolith is hyperactive
Epley’s manoeuvre is done for which scc?
Posterior scc
If problem is in right posterior scc then start with right side turning of head
Manoeuvre for horizontal scc
Barbecue roll / gufoni manoeuvre
