Acute Suppurative Otitis Media Flashcards
What is acute suppurative otitis media ? (ASOM)
Acute inflammation of the middle ear cavity by pyogenic organism
What is middle ear cleft ?
Eustachian tube
Tympanic cavity/middle ear cavity
Attic
Aditus
Antrum
Mastoid air cells
What is the most common route of spread of ASOM?
Via Eustachian tube - ASOM usually follows upper respiratory tract infection caused by viral infection followed by super infection by pyogenic organisms
Routes of spread of ASOM
1.via Eustachian tube - most common
2.via external ear - through perforation of tympanic membrane
3.via blood
Most common organism causing ASOM
Streptococcus pneumoniae
H.influenza
M.catarrhalis
Stages of ASOM
1.stage of tubal occlusion
2.stage of pre-suppuration / exduation
3.stage of suppuration
4.stage of resolution
5.stage of complications
Treatment of ASOM
1.antibiotics
2.decongestant nasal drops
3.oral nasal decongestants
4.analgesics and antipyretics
5.ear toilet
6.dry local heat
7.myringotomy
Myringotomy indications
1.Stage of suppuration
2.Effusion beyond 12 weeks
3.Not resolving even though antibiotics are used
